Selecting specific display when rendering fullscreen

Hi,

attached is a patch for SDL 1.2.13 (for possible inclusion in 1.2.14)
that allows fullscreen on a specific display on the Mac. Using this
patch, setting the environment variable SDL_VIDEO_FULLSCREEN_DISPLAY
to the index of the display to use will cause sdl to use only this
display when rendering fullscreen.

The patch is by Marco Kuhn (marcisimo at googlemail.com).

I checked in an improved version of this patch, but it’s disabled
because mouse events need to take the screen position into account.

Can you look into that?
